Indications

Warfarin is used to prevent and treat thromboembolic disorders, such as:

  • Deep venous thrombosis (DVT) and pulmonary embolism (PE),

  • Prevention of clotting in atrial fibrillation, post‑myocardial infarction, or with mechanical heart valves
     

Instructions for Use

  1. Dosage:

    • Initial dosing often 10 mg daily for 3 days, then adjusted per INR (target 2.0–3.0; 2.5–3.5 for mechanical valves).

    • Lower starting doses (5 mg daily) for elderly, low body weight, or high initial INR patients. 

  2. Administration:

    • Swallow whole with or without food, at the same time each day. Avoid crushing or chewing. 

  3. Monitoring & Adjustments:

    • Regularly measure INR, daily during initiation and then weekly to monthly once stable; monitor more frequently if interacting meds are started/stopped. 

    • Discuss warfarin management before surgeries, dental procedures, or when adding new medications.

  4. Precautions:

    • Many drug and food interactions—antibiotics, NSAIDs, herbal supplements (e.g. St. John’s wort), and Vitamin K-rich foods (e.g. leafy greens) can alter INR significantly.

    • Alcohol increases bleeding risk.

    • Contraindicated in pregnancy (esp. 1st trimester); use with caution in hepatic/renal impairment and conditions with bleeding risk.
