Instruction

• Dosage: Adults: 125 mg every 12 hours, may adjust by 125 mg based on therapeutic response or serum levels (target 10–20 µg/mL); Children (2–25 kg): 10 mg/kg every 12 hours 

• Administration: Swallow tablets whole with food—do not chew, crush, or chew—to maintain sustained-release properties
