Indications:

• Treatment of type 2 diabetes mellitus when diet/exercise (± metformin) are insufficient

• Prevention of heart failure hospitalization and CV mortality in adults with heart failure (NYHA II–IV, reduced ejection fraction)

• Treatment of chronic kidney disease in adults
   

Instructions:

• Standard adult dose is 10 mg once daily, with or without food

• Can be used alone, with other glucose-lowering agents including insulin, or as initial therapy with metformin

• No routine dose adjustment required for elderly or mild-to-moderate renal/hepatic impairment; consult physician for severe impairment
